The Children's Long-Term Support (CLTS) Waiver Program in Wisconsin is a Medicaid program that provides services for children with disabilities. The program helps children and their families live in their homes and communities.
Eligibility
Children must be younger than 22 years old
Children must be eligible for Wisconsin Medicaid
Children must live at home, in foster care, or in another approved setting
Children must need a level of care that is similar to a hospital, nursing home, or institution for people with developmental disabilities
Children must be able to receive safe care at home or in the community
Families can apply for and receive services through their local health and human services department. A support and service coordinator (SSC) will work with the family to help them navigate the program.
